Birch (Betula) is considered one of the clinically most significant tree allergens in Central Europe. Its pollen is exceptionally aggressive — in sensitive individuals, symptoms are triggered by just a small number of grains per cubic meter of air, far fewer than what is needed for grass or ragweed pollen. That is exactly why the spring months bring the worst weeks of the year to many allergy sufferers who otherwise have few problems.

How to recognize birch

Two species are most common in Bosnia and Herzegovina: silver birch (Betula pendula), recognizable by its white, flaking bark and drooping branches, and Betula pubescens, which grows mainly in wetter and higher-elevation locations. Both belong to the birch family (Betulaceae), which also includes alder.

Birch is a fast-growing, light-loving tree that often appears as a pioneer species along forest edges and clearings, while also being a popular ornamental tree in parks, gardens, and along streets. This widespread presence — from natural forests to urban plantings — means birch pollen is practically impossible to avoid, even if you live in a city center.

Flowering season and distribution in Bosnia and Herzegovina

Birch flowers in Bosnia and Herzegovina from April to May, with concentrations usually peaking in the second half of April. Flowering is relatively short and intense — within a few weeks the tree releases enormous quantities of pollen, which wind can carry several hundred kilometers.

Birch is present throughout Bosnia and Herzegovina, and is especially prominent in:

  • the Sarajevo basin, where the forested hills around the city contribute large amounts of pollen,
  • mountain and highland areas and forests, for example Bjelašnica, Vlašić, and the mountains of eastern Bosnia,
  • forested areas of northern Bosnia, for example around Majevica and Kozara.

At higher-elevation sites the season typically starts later and is shorter, as cooler spring weather pushes flowering into May.

Symptoms of birch pollen allergy

Birch is considered one of the most potent tree allergens — many sensitive people react even at very low concentrations that would not yet trigger symptoms from other pollen. Typical symptoms include:

  • sneezing fits, watery nasal discharge
  • nasal congestion
  • red, itchy, and watery eyes
  • itching of the palate and throat
  • worsening of asthma in asthmatics
  • fatigue and reduced concentration during the peak of the season

In some people, symptoms first appear in adulthood, with no prior history of allergies in childhood.

Cross-reactivity with food — the birch-fruit-vegetable syndrome

Birch is known for its pronounced cross-reactivity with plant-based foods. The main culprit is the protein Bet v 1, which belongs to the PR-10 protein family and is structurally very similar to proteins found in many fruits, vegetables, and nuts. The result is what is known as oral allergy syndrome (also called birch-fruit-vegetable syndrome), which in some birch allergy sufferers appears after eating:

  • apples, peaches, cherries, plums, apricots — most commonly
  • hazelnuts
  • carrots and celery
  • soy (especially fresh soy products)

Typical signs are itching and tingling in the mouth and mild swelling of the lips or tongue shortly after eating, and more pronounced reactions occur less often. Because Bet v 1 is a heat-sensitive protein, cooked, baked, or preserved food (stewed fruit, jams) usually does not cause symptoms.

Cross-reactivity with other pollen

Because birch belongs to the birch family, there is also significant cross-reactivity with alder pollen, which flowers early in spring and contains a related protein. People allergic to alder are often also sensitive to birch (and vice versa), so their symptoms can stretch across a longer part of spring.

Treatment and symptom relief

The approach is similar to other pollen allergies, but because birch has a short and intense season, starting treatment on time matters:

  • second-generation antihistamines, taken regularly throughout the season
  • nasal corticosteroid sprays, which it makes sense to start using 2–3 weeks before the expected start of the season
  • eye drops for allergic conjunctivitis
  • for asthmatics, bronchodilators and inhaled corticosteroids as directed by a physician

Immunotherapy

Specific immunotherapy against birch pollen (SCIT — subcutaneous injections, or SLIT — sublingual drops or tablets) is a well-established and clinically proven method of long-term treatment. It reduces both the intensity of seasonal symptoms and the need for medication, and in some people also eases cross-reactions to related foods. Treatment usually lasts 3–5 years and is initiated by an allergist.

Practical tips for allergy sufferers

  • During the peak of the season (second half of April), follow daily pollen levels and adjust outdoor activity timing where possible.
  • Ventilate early in the morning or late in the evening, when concentrations are lower.
  • Avoid drying laundry outdoors during the peak of the season.
  • If your mouth itches after eating apples, peaches, or hazelnuts, it is worth getting tested by an allergist — this may be cross-reactivity rather than a standalone food allergy.
  • On days with high concentrations, rinse your face and hair after coming home, since pollen sticks to them.
  • A short trip to Herzegovina during the peak of birch season can bring a few days of relief, since birch is less widespread there.
